  • Schedule Your Appointment: Once you've decided to make an appointment, you'll be directed to our scheduling platform where you can choose the most convenient time for you.

    If you prefer to speak to a live representative, we've got you covered! Simply call our toll-free number for assistance and support.

  • Easy-to-Fill Out Forms:  Prior to your visit, you'll receive simple digital forms that you can fill out on your smartphone, tablet or computer.  The medical history forms are crucial for your first visit.  Filling these out in advance will ensure Dr. Katz can dedicate more time to discussing your treatment plan.

    Required Forms: Please be sure to sign the HIPAA form, Telehealth consent, and the Responsibility for Payment form. Your cooperation ensures a smooth and efficient appointment.

    We accept traditional Medicare but do not participate with Medicare Advantage plans or other insurance plans. 

    You will be provided with a detailed bill that you can submit to your insurance company or use a flexible spending account, if you have one.   We do not submit these forms to your insurance company and we can not guarantee that your insurance company will cover the cost of the visit.

  • Upon Completion of Forms: Once you've filled out and signed all necessary forms, you'll receive a notification confirming your appointment.

    Reminders: You'll also receive reminders via text, email, or both, based on your preference. Please note fees may apply to text messages.

    These notifications and reminders ensure you're well-prepared and informed for your upcoming telehealth appointment with Dr. Katz.

  • Diagnostic tests: You'll be provided with a script for blood work and asked to record your blood pressure and heart rate. These tests are essential for assessing your current health status.

    Duration: Your initial consultation will last up to 60 minutes, ensuring ample time for thorough discussion and planning

3. Follow Up Visits

  • Based on your individual progress and needs, we'll work together to make any necessary adjustments to your medication regimen and explore additional techniques that may support your weight loss journey

    Our goal is to provide you with comprehensive care and support every step of the way, ensuring a positive and successful health journey.

  • Once you've completed the blood work and provided your blood pressure and heart rate, we'll collaborate to determine the safest and most effective medication options to help you achieve your health goals.

    Duration: Your follow up visit will last up to 30 minutes, ensuring ample time for thorough discussion and planning
